A collaborative of hospitals, FQHCs, and community agencies in Sonoma County has come together to better coordinate care for the people that they serve. The project is funded by the Blue Cross Blue Shield Foundation of California, with Center for Care Innovations convening the members of the collaborative. Seth Emont, a former program officer at Robert Wood Johnson Foundation, is leading the evaluation process.
All members of the Sonoma County collaborative use Purple Binder to find resources and make secure referrals for their patients. The collaborative’s member organizations work together to address the social determinants of health.
The Sonoma County collaborative uses Purple Binder’s secure, HIPAA-compliant messaging system to close the loop on patient referrals. Data from the referrals is used to evaluate outcomes for the collaborative.
