PArTNER (Patient Navigator to Reduce Readmissions) is a randomized controlled trial (RCT) examining the effect of patient navigators on readmissions. The study is a randomized controlled trial led by Dr. Jerry Krishnan, Associate VP of Population Health at University of Illinois Health & Sciences System.
“Ultimately, by allowing them to take better care of themselves, we aim to keep patients healthier and out of the hospital.”
–Amelia Mutso, PArTNER Project Manager

PArTNER’s patient navigators use Purple Binder to address the non-medical barriers of study enrollees, including transportation, food security, and housing — all of the social determinants of health.
One of the first RCTs to examine the effect of social determinants of health on readmissions, PArTNER is using Purple Binder to collect granular data about the community referrals made by navigators to address patient barriers to health.
