CHECK is a pediatric care coordination program for kids with complex medical conditions. CHECK uses Purple Binder to connect patients and their families with the resources they need to thrive. CHECK’s community health workers use Purple Binder in the field and in the office.

CHECK uses Purple Binder’s reporting to look at needs across its entire patient population. CHECK’s community health workers are equipped with hyperlocal information that connects patients and their families with the resources they need. CHECK is using Purple Binder to securely coordinate care with a network of 50 community-based partners.

CHECK uses data collected through Purple Binder in its reporting to CMS and coordinates care with community agencies to better serve patients and their families.
